Discusses the problems encountered in implementing primary health care under the circumstances imposed by structural adjustment programmes (SAPs) and the approaches developed in response. A description of how economic decline and SAPs contribute to impoverishment and their relation to delivery and access to health care are provided. The effects of SAPs on PHC policies are explored debating their contribution to the dissatisfaction by populations with health sectors in most countries. Among other issues discussed is the need for a frame of reference for the organisation of health services in the process of coping with SAPs.
